New Patient Forms
Welcome to the website of M. Catherine Talmo, DMD. If you are a new patient, we ask that you fill out the Health History Form below before you visit with us. You can download the form, fill it out electronically and submit it via the secure email address info@talmodmd.com. Another option is to print the form, fill it out and bring it with you.
Completing this form before your visit allows us to attend to your dental needs faster and more efficiently. If you have any questions, please call our office at 617-864-6111.

Patient Forms
Insurance
Dr. Talmo participates with Delta Dental Premiere and PPO Plus Premiere plans. M. Catherine Talmo DMD also participates with Blue Cross / Blue Shield.
For all other insurance plans, we gladly submit insurance claims on the patient’s behalf. Please note that we cannot submit claims for patients covered by MassHealth.
We ask that you always check with your insurance carrier to verify that you are not restricted to a closed network of providers.
